You may complete the following forms if you choose, or you may wait to fill out all of the necessary forms at your first appontment. Please arrive a few minutes early to your first appointment so that you may complete your initial paperwork in a timely way.

        If you would like for me to coordinate care with another provider for you (for example, your physician, endocrinologist, etc.), please complete this form. If it will be more convenient, you may complete the Consent to Release Information Form at a later date in our office.
