Confidentiality and Privacy Policies


        You and the other patients in our care will generally be provided with a Notices of Privacy Practices handout during the initial visit. This document describes how Diversified Family Interventions will handle your Protected Health Information (PHI) in detail. It informs you how we use this PHI in our practice, how we share your PHI with other professionals and organizations and how you may access it. We are mandated to inform you about this because of the privacy guidelines of the Health Insurance Portability and Accountability Act (HIPPA) of 1996. Additionally, it is important for us to inform you of this so that you may make the very best decisions for yourself, your family and/or other significant parties that you may represent. 

       Until you visit our office and receive a Notices of Privacy Practices handout, we want you to fully understand that the law protects the relationship between a patient and a psychotherapist (and/or physician), and information may not  be disclosed without the patient's express written permission. Primary exceptions to this may include:

  • Suspected child abuse, dependent adult or elder abuse for which I am required by law to report to the appropriate authorities immediately;
  • A client threatening serious bodily harm to another individual requires me to notify the police/legal authorities and inform the intended victim as soon as possible;
  • A client's intentions and plans to harm himself or herself will generate my efforts to enlist their cooperation for their safety and well-being. If they refuse to cooperate, I may take further measures, without their consent, as provided to me by law in order to ascertain their survival and safety.
